Introduction

Welcome, whether you are new to healthcare or completing your annual refresher

This course is written for TWO audiences:

  •  First-timers: Every concept is explained from scratch with plain language,

    real examples, and step-by-step guides.

  •  Annual refreshers: Yellow Refresher Reminder boxes highlight where practice

drifts over time, updated guidance, and honest reflection questions.

Duration: approximately 1.5-2 hours   |   Pass mark: 70%

Read through each chapter as if your trainer is speaking directly to you.

If It Is Not Written Down, It Did Not Happen

That sentence sounds harsh. But in healthcare law, clinical audit, safeguarding investigations, and professional fitness-to-practise hearings, it is functionally true. Records are not bureaucratic add-ons. They are the clinical evidence of the care you provided, the observations you made, and the decisions you took.

For first-timers: Good record-keeping is a clinical skill that takes time to develop. This course gives you the foundations and the standards to aim for.

For refreshers: Record-keeping is one of the areas most vulnerable to shortcuts when workload is high. How honest would you be if a lawyer asked you whether your documentation from last week fully reflected the care you provided

Poor record-keeping is one of the most common findings in NMC fitness-to-practise cases.
It is cited in coroner's inquests, safeguarding inquiries, and clinical negligence claims.
Your documentation represents you when you are not in the room.

Objectives
By the end: understand why records matter legally and clinically; apply the principles of good record-keeping; understand data protection obligations; complete different types of healthcare records correctly; report concerns through the right channels; and recognise records as tools of accountability and learning.
