This course is written for TWO audiences:

    drifts over time, updated guidance, and honest reflection questions.

Records are not bureaucratic add-ons. They are the clinical evidence of the care you provided, the observations you made, and the decisions you took.

For first-timers: Good record-keeping is a clinical skill that takes time to develop. This course gives you the foundations and the standards to aim for.

For refreshers: Record-keeping is one of the areas most vulnerable to shortcuts when workload is high. How honest would you be if a lawyer asked you whether your documentation from last week fully reflected the care you provided
